What is the Herpes Zoster (Shingles)?

The viral infection that is caused by the reactivation of the VZV i.e. varicella-zoster virus, the same virus that causes chickenpox, is known as herpes zoster (Shingles). This infection presents in the individual as painful blisters, burning sensations, rashes, fatigue, tingling sensations, etc. on one side of the body, particularly in the dermatome (nerve pathway). This virus remains in a dormant state in the nerve cell after recovering from the chickenpox disease and reactivates in later life especially when immunity is low in the body.

What is the Herpes Zoster (Shingles) – As Per Ayurveda?

Herpes Zoster, according to Ayurveda, can be closely correlated with a condition caused by the aggravation of Pitta dosha i.e. Pittaja Visarpa based on symptoms. The symptoms such as burning sensation (Daha), redness (Raga), pain (Toda), and rapid spread of eruptions resemble the classical features of Visarpa.

It can also be correlated with Kaksha, a disease described in Ayurvedic texts as affecting the area between the axilla (Axilla) and flanks, involving severe pain and rashes which is very similar to the dermatomal distribution seen in shingles.

Along with this, it is also correlated with Rakta Pradoshaja Vikara the disease which is caused due to vitiation of blood, and Pittaja Kshudra Roga which are minor ailments caused by Pitta imbalance in the body, due to its involvement of skin, blood, and Pitta dosha. 

Why Does Herpes Zoster (Shingles) Affect One Side of the Body?

Typically, Herpes zoster affects only one side of the body because this virus reactivates from a specific nerve ganglion of the body, which supplies a particular area of skin innervated by that nerve (dermatome) as it remains in a dormant phase on that particular dermatome.

When the virus becomes active again, especially in individuals with low immunity, it travels along the same nerve fibers of that single ganglion, leading to the one-sided blister formation, burning sensation, rashes, and pain.

Who Gets Affected by Herpes Zoster (Shingles)?

Anyone who had a previous infection of chicken pox can be affected by Herpes zoster as the virus remains in their body and may react at any time later in their life. However, certain groups are at higher risk like individuals over 50 years old as with age their immune system weakens.

Patients with conditions like AIDS, HIV, or patients undergoing immunosuppressive treatment, chemotherapy, etc. are more susceptible to being affected by shingles. Stress and lifestyle factors like irregular sleep patterns, etc. may also contribute to the reactivation of the chicken pox virus. Along with this family history of having herpes zoster also increases the chances of having this infection.

How Common is Herpes Zoster (Shingles)?

Most people have shingles once in their lifetime, but herpes zoster can reoccur more than once. 30 % of the population have a lifetime risk of having herpes zoster. As per the data, one out of 3 people in the US are affected by shingles and each year less than 100 people die due to herpes zoster.

Shingles can also occur in children, but they are not as common as in adults. Approximately 10-18% of individuals who suffer from shingles have complication PHN also known as post-herpetic neuralgia.
In one French study, approximately 15% of the cause of death in the shingles are due to HZ encephalitis which was identified as the second most frequent cause of death. In Asia, North America, and Europe, the incidence of shingles is estimated to be 3-5 per 1000 people per year. Recent studies suggested around 705 cases per million of shingles annually in India. 45 years of age. This helps to prevent genital warts and HPV-related cancers.

Types of Herpes Zoster (Shingles)

  1. Zoster Sine Herpete: A less common variant which is also known as shingles without rash. In this type of nerve pain occurs but without the characteristic skin rash, making its diagnosis more challenging without laboratory tests.
  2. Zoster simplex (Localized herpes zoster): The common type of herpes zoster is zoster simplex, which affects the area of skin supplied by a single spinal nerve also known as a dermatome. In this type of herpes zoster blisters, pain, and rashes all are limited to only one side of the body.
  3. Herpes Zoster Ophthalmicus: This type of herpes zoster involves the ophthalmic branch of the trigeminal nerve which affects the upper eyelid, forehead, eye, etc. In Herpes Zoster Ophthalmicus serious eye complications can occur including vision loss of the individuals.
  4. Ramsay Hunt Syndrome (Herpes zoster oticus): This syndrome involves the auditory and facial nerve of the individual near the ear which leads to ear pain, vertigo, hearing loss, and even facial paralysis.
  5. Herpes zoster multiplex: It is a rare type of herpes zoster in which more than one dermatome is affected and is mostly seen in immunocompromised individuals. In this type, widespread and severe lesions occur.
  6. Herpes Zoster disseminated: This type of herpes zoster occurs mostly in people with weak immune systems and mainly resembles generalized chicken pox.  In this type of herpes zoster, a lesion appears outside the affected dermatome area.

Causes of Herpes Zoster (Shingles) – As Per Ayurveda

  • Vitiation of Pitta Dosha: The vitiation of Pitta Dosha can occur due to excessive consumption of sour, salty, pungent food (Amla, Lavana, Katu Aahara). Along with this excessive intake of hot (Ushana) and sharp (Tikshana) foods.
  • Prolonged exposure to heat or sunlight (Atapa).
  • Emotional imbalance like Anger (Krodha)
  • Association of Vata Dosha (Due to Vata Anubandha) which occurs due to excessive travel, anxiety (Chinta), fasting (Upvaasa), and irregular sleep (Ratri Jagrana, Dwiva Swapana). It also occurs due to excessive intake of dry (Ruksha), and cold (Sheeta) foods and due to excessive Vata-increasing activities.
  • Low Digestive Fire (Manda Agni)
  • Toxin Formation (Ama Uttapati) occurs due to irregular eating, intake of heavy meals (Guru Bhojna), and eating incompatible foods (Viruddha Ahara).
  • Vitiation of Blood (Rakta Dushti) due to toxin (Ama) formation, use of Visha Yukta Aahara- Vihara (toxic food and habits).
  • Psychological Causes (mansika Nidana) are worry (Chinta), grief (Shoka), and fear (Bhaya).
  • Weaken immunity (Oja) due to aging, chronic diseases (Jeerna Vyadhi), post-infection state, and allowing latent doshas and infections to surface.

Symptoms of Herpes Zoster (Shingles) – Modern Aspect

  • The first symptom of shingles in most cases is the feeling of a band-like area on one side of the body or face. In that area, throbbing, stabbing, or sharp pain occurs.
  • On the affected area tingling sensation or numbness can be felt.
  • Usually after 1-5 days of pain in the nerve distribution area i.e. dermatomal pattern rashes appear on one side of the body.
  • After that, these rashes turn into small painful blisters which make clusters.
  • Within 7-10 days, these blisters ooze, burst and form crusts.
  • Skin becomes sensitive to touch.
  • Mild fever

  • Headache
  • Postherpetic neuralgia
  • Herpes zoster ophthalmic affects the eye and causes vision problems.

Diagnosis of Herpes Zoster (Shingles) – As Per Ayurveda

Diagnosis of herpes zoster is made by inspection (Darshan) in which reddish eruptions, clustered vesicles, and one-sided rashes are observed. After that palpitation (sparshana) is done to check tenderness, sensitivity to touch, etc. then interrogation (Prashana) is done on the patient.

Tenfold examination (Dasha-Viddha Pariksha) is also done in herpes zoster patients. Along with this if required, lab tests like the Tzanck smear test, and PCR. DFA test, VZV serology, and skin biopsy will also be done.
