Ear Wax Blockage Associated Problems

The ear wax or cerumen is a brown or yellowish waxy oily substance produced by ceruminous glands which are modified apocrine glands in the exterior auditory canal. Ear wax does the function of protecting the ear from dust, foreign bodies, microorganisms etc. Ear wax contains keratin, long chain fatty acids and cholesterol. It is slightly acidic with antibacterial properties. In normal physiological conditions, ear wax secretion in ear is under normal limits without any hardening or ear clogging. Excess accumulation of ear wax leading to ear infection may cause numerous ear disorders of varying severity.

Etiology

Ear wax blockage is usually caused by impaction i.e. pushing ear wax deep within ear canal by using cotton swabs or other articles in attempts to remove ear wax, hearing aids and ear plug users are also at greater risk of disorders due to ear wax blockage

Diagnosis

Diagnosis is usually clinical by history of complaints and examination of ear canal by otoscope.

Management

Ear wax blockage is usually managed by removing the ear wax by scooping it out by small instrument called curette. Appropriate antimicrobial drugs are administrated in case of infection. Ear is irrigated with warm water, sodium bicarbonate, or other ear drops before scooping out the hardened ear wax. Gentle section is provided if necessary. Removal of ear wax without taking medical help may worsen condition like infection or canal perforation leading to much serious ailments such as permanent hearing loss , middle ear infection etc.

Complication

Improper or failure to remove ear wax build up may cause perforated ear drum, permanent hearing loss, middle ear Infection etc.

Ayurvedic Management

Ear wax blockage can be associated with mala vriddhi (increased waste products of metabolism) and kapha increase in vata dosha site. The external ear canal can be considered as vata dosha site. As the excessive production or defective drainage of cerumen from ceruminous gland can be correlated as increase in unctuousness, stickiness, and  stable quality of kapha, symptom are produced as per function of vitiated kapha. Treatment Principle

As the vitiated dosha is kapha, kapha dosha reducing and channel cleansing treatment have to be followed by using ushna (hot potent), tikshna dravya prayoga (penetrating as well as rapid acting medications) with predominance of bitter, acid and astringent taste usage.
